Available DIFN Forms:

  • Client Intake: This form is used to gather essential information from new clients seeking our services. It helps us understand your needs, background, and how we can best assist you. Please provide accurate and complete details to ensure a smooth and efficient process as we begin working together.

  • Community Service: This form is used to record and verify community service hours for [specific purpose, e.g., school requirements, court-ordered service, or volunteer certification]. Please provide accurate details of the service performed, including dates, hours, and the organization or event where the service was carried out.

  • Donation Request: This form is for individuals or clients in need of assistance. If you are facing hardships and require support, please provide detailed information about your situation, the specific assistance you need, and how the donation will help. Our team reviews all requests to ensure that support is provided to those who are most in need. (At this time we do not provide financial assistance)

  • Hot Meal Delivery: This form is for individuals in need of hot meal delivery services. If you or someone you know requires assistance with meal delivery due to age, illness, or other challenges, please provide the necessary details, including delivery address, dietary restrictions. Our team will review your request to be included in the program and arrange for meals to be delivered. The Client must live within10 Mile radius of 4103 Monument Avenue Richmond VA 23230.

  • Divine Gifts of Joy Christmas Registration: This form is for families who wish to receive holiday gifts through our Divine Gifts of Joy Christmas program. Please provide details about the recipients, including ages, gift preferences, and any special needs. Our team will review your submission to ensure that everyone receives the joy and support they deserve this holiday season.
